RN Goody Reede of Broughton 63. yeres old sick very extreme sick on satterday last leekely to have dien being the 7. of octob. & then recovered again & fell sick agayne vppon tuesday after. being the 10 of Octob. sent to me vppon thursday being the 12 of octob. it taketh all above the gyrdell first in the shoulders to the brest & so to the hart her sydes wher it so lyeth as that on may tak out thenc as shee thinketh 1598. h die h. 8. 30. mi. 1598.
